NOTICES

• Payments are stored in a secure external Payment Processing System utilized for appointments.

• Payment information is not accessible by therapist’s or others, once submitted or entered into the Payment Processing System.

• Payments will be automatically charged on the date of the appointment at midnight.

• Appointment length is 45 minutes but may go up to 50 minutes.

• Telephone conversations, site visits, writing and reading of reports, consultation with other professionals, release of information, reading records, longer sessions, travel time, etc. will be charged at the same rate, unless indicated and agreed upon otherwise.

• Please notify your therapist if any problems arise during the course of therapy regarding your ability to make timely payments.

• 48 Hours - Cancelation Notice:

  • 48 Hours Notice - is required for all cancelations or changes to appointments.
